History: Ollie presented for evaluation of severe erosions, ulcerations, crusting and intense pruritus affecting the periorbital region, lip margins, perineum, ventral tail base and paws.

Test /Procedure: Skin biopsies were collected from all sites for histopathologic evaluation. . Cytologic evaluation of the skin surface revealed numerous bacteria, degenerate neutrophils and macrophages. A surface skin swab was collected for bacterial culture and sensitivity confirmed a moderate growth of Pseudomonas aeruginosa.

Diagnosis: Mucocutaneous lupus erythematosus with secondary bacterial overgrowth

Treatment: immunomodulatory: oral prednisolone and cyclosporin.  The lesions were regularly cleaned with sterile saline and non-perfumed baby wipes, followed by application of topical silver sulphadiazine

Prevention or Follow up: Within five days of starting this treatment, there was complete resolution of skin lesions. No irritation or discomfort was noted from the affected areas. The immunomodulatory treatment will be tapered with the intent to withdraw the prednisolone and reduce the cyclosporin administration. Serum trough levels of cyclosporin have been monitored for toxicity.
